5. Submit a Form DS-260
As soon as your application is successful, submit online a Form DS-260. If you were married or had a child after you submitted your application, you can add them with accompanying paperwork. If you did not include an existing spouse or children before applying, you will be disqualified.
On the sign and submit page, enter your DV case number without zeros (DV case number is 2018AF0000012345, then enter ‘2018AF12345).
Finally, print the confirmation page and bring it to the interview.
