If you have previously been deported from the US, submit form I-212.
You must supply marriage certificates and marriage termination (including by death) documents, where applicable. If you have served in any military services, you must bring documentation.
You must also supply a police certificate that covers the entire period of your residence and any arrests.
Any adopted children must submit certified copies of the adoption decree and legal custody decree.
There is a reciprocity option for marriage licenses, military service, and police certificates.
